New Change in Learning Method Form

A new change in learning method form is now available.
This form is ONLY to be completed by parents/guardians who wish to change their choice of learning method for the November 16 re-entry date.
Families who wish to continue with their current choice of learning method do NOT need to fill out a form or take any additional steps.
Your selection on this form will be your child's method of learning until the second school re-entry date on February 1, 2021.
Please complete one form for each child by Tuesday, October 20.
