Parent Survey: Online Learning for the 2021-2022 School Year

The Google Form that will be used to determine the level of interest in online programming for the 2021-2022 school year is now open. The form is available in three languages: English, Spanish, and Tagalog.
If you hope to enroll your child in online learning next year, please follow the links below to complete your preferred form. Please do NOT complete a form if you plan to have your child attend school in-person.
English: Online Learning for the 2021-2022 School Year (June 18 to July 28)
Tagalog: Online Learning Para sa 2021-2022 School Year (June 18-July 28)
These forms will be open from June 18th until July 28th. After the July 28th deadline, the responses received will be considered final. Should an online program be offered, families who chose to have their children learn online must do so until February 1, 2022. Switching back and forth between learning methods will not be permitted.

If you choose to complete this Form, you will receive an emailed copy of your responses after submitting. If you change your mind about your response before July 28th, you may edit your response by referring to this email and clicking the “Edit Response” button.
